Required documents for IVF/IUI application

In order for the process of in vitro fertilization (IVF) or Intrauterine insemination (IUI) to go smoothly, couples should be fully equipped with knowledge, psychology and do not forget to prepare full documents when deciding to choose this method. There are many couples who are still not clear on the issue of administrative paperwork, which leads to slow procedures, prolonging the waiting time before starting IVF.

At the AF Hanoi, in order for the IVF/IUI to be performed quickly and conveniently, all you need to do is prepare the following documents when you come to the hospital:
  1. Identity card or identity card of the couple (original): The hospital will only keep a copy and return the original after comparison.
  2. Marriage registration (original): The hospital will only keep a copy and return the original after comparing
  3. Fill out the application form for IVF/IUI technique: The application form is available at the Records Department – AF Hanoi
Each customer when treated at AF Hanoi will be given a patient code, which is used at all stages of examination and testing; Along with that, customers will take fingerprints and take photos to perform 3-layer security, thereby avoiding all possible confusion during the IVF process later.
